PROCEDURES IN APPLYING FOR CERTIFICATE OF COMPLIANCE

Procedures in Applying for Certificate of Compliance (COC)

Mga Hakbang sa Pag-aaply ng Certificate of Compliance (COC)

  1. Secure the application form and checklist of appropriate requirements for COC application at the Licensing & Market Monitoring DIvision (LMMD) located at the 12th Floor or through www.erc.gov.ph:

    Normal processing time: 5 minutes

    Kumuha ng COC Forms at checklist ng mga kaukulang requirements sa pag-apply ng COC sa Licensing & Market Monitoring Division (LMMD) na nasa 12th Floor o hanapin at i-print ang forms at checklist sa www.erc.gov.ph. Normal na tagal ng pagproseso: 5 minuto

  2. Accomplish the forms and submit together with the appropriate requirements in the checklist, in two (2) hard copies and one (1) soft copy (diskette or CD), to any LMMD Officer at the 12th Floor.

    Normal processing time: 10 minutes

    Kumpletuhin ang mga forms at ang iba pang dokumentong nakatala sa checklist at ibigay ang dalawang (2) kopya at isang (1) soft copy (diskette or CD) sa nakatalagang LMMD Officer sa 12th Floor.

    Normal na tagal ng pagproseso: 10 minuto

  3. Secure assessment form in five (5) copies from the LMMD Officer

    Normal processing time: 5 minutes

    Kumuha ng limang (5) kopya ng Assessment Form sa LMMD Officer

    Normal na tagal ng pagproseso: 5 minuto

  4. Proceed to the Cashier at the 14th Floor for payment of appropriate COC application fee in accordance with the fees and charges, as amended. The Cashier shall give the applicant three (3) copies of the Assessment Form and an Official Receipt (O.R.).

    Normal processing time: 10 minutes

    Magtungo sa Cashier na nasa 14th Floor upang magbayad ng kaukulang COC application fee ayon sa fees and charges, as amended. Ibabalik ng Cashier sa aplikante ang tatlong (3) kopya ng Assessment Form at ang resibo o Official receipt (O.R.).

    Normal na tagal ng pagproseso: 10 minuto

  5. Proceed to the LMMD Officer at the 12th Floor and submit two (2) copies of the Assessment Form duly signed by the Cashier and show proof of payment i.e., Official Receipt for verification purposes.

    Normal processing time: 1 minute

    Bumalik sa LMMD Officer na nasa 12th Floor at ibalik ang dalawang (2) kopya ng Assessment Form na pirmado ng Cashier at ipakita ang resibo bilang katibayan ng pagbabayad.

    Normal na tagal ng pagproseso: 1 minuto

Note:

  • The entire processing for filing of application for COC will take more or less 30 minutes.
  • No application shall be processed for evaluation unless all documentary requirements and proof of payment of the Application Fee shall be submitted and presented by the applicant to the LMMD Officer.
  • Upon submission of all forms and those listed in the checklist of requirements for COC application, wait for the Commission’s advice of its action within thirty (30) days for PEZA-registered entities and sixty (60) days for all other applicants.

Paalala:

  • Ang kabuuang proseso sa pag-aaply ng COC ay tatagal ng humigit kumulang sa tatlumpung (30) minuto.
  • Walang application ang ipro-proseso para sa COC hanggang hindi naibibigay ng aplikante sa LMMD Officer ang kumpletong forms at dokumento kasama ang Official Receipt na katibayan ng pagbabayad ng Application Fee.
  • Maghintay ng tatlumpung (30) araw para sa PEZA-registered na kumpanya at animnapung (60) araw para sa iba pang aplikante upang malaman ang aksyon na ginawa ng Komisyon.
